The SD10 Accessibility Committee has developed questionnaires that aim to gather information regarding experienced or perceived accessibility issues or challenges that may exist in SD10. The committee will use the information obtained through these questionnaires to help inform priorities and recommendations for the District to help advance accessibility in SD10, as well as to finalize the District’s Accessibility Plan. Persons who have experienced or witnessed accessibility issues or challenges are encouraged to complete the following questionnaire. This form will remain open until January 19, 2024.
